LEO Pharma has agreed to acquire worldwide rights to dersimelagon, an investigational once-daily oral melanocortin 1 receptor (MC1R) agonist, from Tanabe Pharma. The deal adds a late-stage rare dermatology asset for erythropoietic protoporphyria (EPP) and X-linked protoporphyria (XLP) to LEO Pharma's pipeline.1

Dersimelagon has completed phase 3 development, and a new drug application (NDA) for EPP and XLP was submitted to the US Food and Drug Administration (FDA) in June 2026. If approved, it would become the first oral therapy for EPP and XLP.1

The acquisition builds on LEO Pharma's expansion into rare dermatology, including its 2025 partnership with Boehringer Ingelheim for spesolimab (Spevigo) and its acquisition of Replay's herpes simplex virus gene therapy platform.1

Phase 3 INSPIRE Trial Supports Dersimelagon NDA

Dersimelagon was evaluated in the phase 3 INSPIRE trial (INcreased Sun Exposure Without Pain In Research Participants With EPP or XLP; NCT06144840), a global, randomized, double-blind, placebo-controlled study.2

Results announced in March 2026 showed dersimelagon met the study's primary and secondary end points, with statistically significant and clinically meaningful outcomes. The findings were also presented as a late-breaking presentation at the 2026 American Academy of Dermatology Annual Meeting.1,3

Among the reported functional outcomes, dersimelagon significantly prolonged average daily sunlight exposure time to first prodromal symptoms. Exact effect sizes and P values were not provided in the LEO Pharma announcement.

Dersimelagon is designed to increase melanin in the skin, helping reduce sunlight penetration and protect against phototoxic reactions. EPP and XLP are rare genetic disorders characterized by severe reactions to sunlight that can include skin pain, rash, swelling, redness, and burning sensations. Liver damage can also occur in some patients.1

FDA Review Underway

Dersimelagon has received FDA fast track and orphan drug designations for EPP and XLP. An NDA for both indications was submitted to the FDA in June 2026. The therapy has not been approved by the FDA or another regulatory agency, and its safety and efficacy have not been established by a regulatory authority.1

The acquisition could also mark a broader shift in the treatment landscape for EPP and XLP, where emerging therapies are beginning to expand options for patients with rare phototoxic disorders.

Under the acquisition agreement, LEO Pharma will acquire worldwide rights to dersimelagon for up to $435 million in upfront and near-term milestone payments, along with potential downstream milestones and tiered royalties on net sales.¹
