Balancing Current Treatment Options and Adverse Effects
Zeichner noted that several medications currently available can benefit patients with male pattern hair loss, but some are associated with potential adverse effects.
“Despite being so common, we don't have that silver bullet just yet to treat male pattern hair loss,” he emphasized.
Treatment selection therefore requires consideration of the individual patient's needs, risks, and preferences, making consultation with a board-certified dermatologist important. He also highlighted ongoing development of more targeted therapies as a promising direction for improving treatment options.
Disrupting DHT and Androgen Receptors
One particularly active area of research involves the interaction between androgen hormones and receptors within the hair follicle. Elevated levels of dihydrotestosterone (DHT) are associated with progressive miniaturization of susceptible hair follicles, a central feature of androgenetic alopecia. Researchers are therefore investigating ways to interrupt the interaction between androgens and their receptors at the level of the hair follicle, with the goal of preserving or improving follicular function.
Drawbacks of Systemic Medications
Zeichner explained that one limitation of currently available oral medications targeting the hormonal component of hair loss is their systemic exposure. Because these medications are taken orally, their effects are not limited to the hair follicle and can affect tissues throughout the body, potentially contributing to hormone-related adverse effects. This has created interest in approaches that can deliver treatment more selectively to the scalp.
The Promise of Topical Drug Delivery
Topical drug delivery represents an emerging strategy. By applying therapies directly to the scalp, researchers hope to achieve an effective concentration at the hair follicle while minimizing systemic exposure. According to Zeichner, this approach could potentially provide the benefits of targeting the hormonal pathways involved in androgenetic alopecia while reducing the risk of systemic hormone-related adverse effects. Although these approaches remain part of ongoing research, the development of more follicle-specific therapies could ultimately expand the treatment landscape for men with androgenetic alopecia.
