Generalized pustular psoriasis (GPP) remains one of the most complex inflammatory dermatoses encountered in clinical practice. Although it accounts for fewer than 1% of psoriasis cases, its unpredictable course, systemic inflammatory potential, and diagnostic overlap with other pustular eruptions make it disproportionately high risk. Delayed recognition can lead to hospitalization, organ dysfunction, and significant patient morbidity.1-2
In a recent Clinical Case Collective discussion moderated by Aaron Farberg, MD, panelists Tina Bhutani, MD, MAS, Christopher Bunick, MD, PhD, and Harrison Nguyen, MD, MBA, MPH, examined the real-world diagnostic and management challenges of GPP. Their discussion highlighted a consistent theme: GPP rarely presents in a uniform or textbook fashion.

How does GPP typically present in practice?
Although often described as diffuse erythema studded with sterile pustules accompanied by fever and systemic inflammation, real-world presentation is far more heterogeneous.
“Depending on when they present in the course, oftentimes those pustules have already dried out. You might just see these collarettes of scale,” Bhutani noted. “It’s not always your textbook ill patient coming in with a fever and pustules all over their body.”
Farberg added that pustules may appear smaller and more yellow on the surface, further complicating recognition. Patients may present at different stages of evolution, making timing a critical variable in clinical assessment.
Do patients with GPP always have a history of plaque psoriasis?
No — and this contributes to diagnostic delay.
“Sometimes these patients have a history of psoriasis, sometimes they don’t, and so that lends [itself to] the challenge of making the right diagnosis,” Nguyen explained.
The absence of known plaque psoriasis should not reduce clinical suspicion when morphology and systemic features align with GPP.
What systemic features should heighten concern?
GPP is not simply a cutaneous disease. It is an autoinflammatory condition with systemic implications.
Nguyen highlighted fever, chills, arthralgias, and joint pain as common features. More severe cases may involve disturbances affecting the liver, kidneys, or cardiovascular system. Vital sign instability may accompany intense inflammatory flares.
Pain, Farberg emphasized, is often the symptom that prompts patients to seek care — and should not be underestimated as a marker of severity.
What are the key diagnostic pitfalls?
One of the most significant challenges is differentiating GPP from acute generalized exanthematous pustulosis, particularly in patients with recent medication exposure.
Thorough medication review and clinical context are essential. A complete skin examination is equally critical.
“You have to have all these patients in a gown and check every square inch of their body,” Farberg stressed.
Although biopsy can support evaluation, the panel emphasized that GPP remains fundamentally a clinical diagnosis, requiring synthesis of morphology, history, and systemic findings.
What triggers should dermatologists be especially mindful of?
Triggers can include infection, stress, pregnancy, and medications. However, systemic corticosteroid withdrawal emerged as a particularly important precipitant.
“One of the things that can flare or cause a rebound of the GPP is withdrawal from the corticosteroids. And we live in, unfortunately, a high–systemic corticosteroid world,” Bunick said.
This underscores the need for caution when initiating or tapering systemic steroids in patients with known or suspected pustular psoriasis.
How significant is the psychosocial burden?
The panel emphasized that disease burden extends beyond physical symptoms.
“Patients feel like they’re walking around with a ticking time bomb,” Bhutani explained. “They just don’t know when it’s going to go off.”
Even between flares, patients may experience substantial anxiety and reduced quality of life. Severe flares often require hospitalization, creating distress not only for patients but also for their families. Comorbid anxiety, depression, arthritis, and cardiometabolic disease further compound disease burden.
Referral to advocacy organizations such as the National Psoriasis Foundation can provide additional patient support and education resources.
How has targeted therapy changed the management landscape?
The approval of spesolimab, an IL-36 receptor antagonist, marks a significant shift in GPP management. Unlike broad systemic immunosuppression, IL-36 inhibition directly targets a key pathway implicated in GPP pathophysiology.
“Prior to spesolimab, it was harder for patients to cope, and there’s a longer period they had to suffer,” Bunick said. “But the advantage of having an advanced systemic therapy is they don’t have to cope too long. They don’t have to suffer too long if we can get them the medicine fast.”
The panel emphasized that therapeutic innovation must be paired with practice readiness — including rapid triage protocols, insurance navigation strategies, and clear escalation pathways.
