Access Challenges with GEP in cSCC

Learn why gene expression profiling in cutaneous SCC can’t yet guide imaging or radiation decisions—and what evidence gaps still limit its use.

This episode, titled “Access Challenges with GEP in cSCC,” features panelists discussing the practical considerations and real-world challenges associated with incorporating gene expression profiling (GEP) into the management of cutaneous squamous cell carcinoma (cSCC). The expert faculty explore how timing, workflow coordination, and multidisciplinary communication can significantly affect the integration of molecular profiling into treatment planning.

The panel discusses the importance of obtaining GEP results early in the patient journey, particularly before referral for Mohs surgery or multidisciplinary evaluation, to help guide decisions regarding imaging, radiation therapy, immunotherapy, and surgical management. The expert faculty highlight how molecular risk information may influence not only treatment selection, but also sequencing care and coordination among dermatologists, dermatopathologists, surgeons, radiation oncologists, and medical oncologists.

Additionally, the discussion examines barriers related to awareness and utilization of GEP in community practice. The panel reviews how increased education surrounding molecular profiling may improve adoption and support evidence-based decision-making in cSCC management. The expert faculty also discuss the financial considerations associated with GEP testing, including patient access, insurance coverage, and broader healthcare system implications. The panel highlights emerging data suggesting that molecular profiling may help optimize allocation of treatments such as radiation therapy and potentially reduce unnecessary healthcare utilization while supporting more personalized patient care.

In the next episode, “Treatment Strategies in cSCC,” panelists will discuss how gene expression profiling and other molecular tools may help refine treatment escalation and de-escalation decisions in cSCC. The discussion will also explore multidisciplinary care planning and personalized management strategies for high-risk patients.
