As more and more patients seek health care knowledge via social media, questions regarding the accuracy of posted content have been raised. Even with the imminent ban in the United States, TikTok has become a source of education for patients, especially in the world of dermatology. A recent study has tested the quality of information shared on this particular platform and found that the dermatological content shared by creators is of low quality overall.

Investigators screened the platform in November 2022 for keywords like “dermatology,” “skin care,” “aesthetician,” “dermatologist,” and “derm physician assistant.” Another study from 2022 found that the top hashtags were similar and included “#skincare,” “#dermatologist,” “#dermatology,” “#skincareroutine,” and “#acne.”

Based on engagement through these searches, over 35 accounts and 17,085 of the app’s informational dermatology videos were analyzed using the DISCERN criteria to evaluate reliability and clarity. This included the app’s top 10 derm profiles and their corresponding top videos. Interestingly, 2.2% of these videos featured TikTok-verified paid sponsorships with dermatologists comprising 65% of these posts, which can raise concerns about conflicts of interest.

Over 50% of these profiles were run by dermatologists with 17.1% being aestheticians, 11.4% being medical clinics, and 5.7% being medical spas. Medical clinics and aestheticians had the highest average engagement scores of the groups (

 < 0.001). Dermatologist-run accounts had significantly more views (

 = 0.04) with 25,942,000 views and 6895.86 comments versus the 4,969,502 views and 2984.22 comments of non-dermatologist-run profiles.

Furthermore, dermatologist-run accounts were more likely to refer to areas of uncertainty and describe specifically how each treatment works while non-dermatologist-run accounts were more likely to assess each treatment’s risks and provide potential alternatives. According to the study authors, “these differing emphases could shape consumer attitudes towards dermatological care, treatment choices, and even trust in professional advice versus influencer-driven content.”

However, all DISCERN scores were very low (34.18 vs 33.46, respectively), and there was no statistically significant difference between the 2 groups. This indicates that the dermatological content in all videos is of low accuracy and quality overall.

“Physicians should be aware of the expanding role of social media as a source of medical information and possible misinformation,” the authors concluded. “Dermatologists should consider if they have an obligation to produce unbiased, ethical, and accurate content.”

These studies demonstrate the rise of TikTok in the dermatology space, which can have beneficial and disadvantageous effects for clinicians and their patients. Future research can further analyze the influence of TikTok and other social media platforms and how it can positively or negatively impact dermatological care.

A study evaluating TikTok's dermatology content found it to be of low quality, with dermatologists and non-dermatologists differing in their content focus. Dermatologist-run accounts had higher engagement but did not significantly differ in content quality from non-dermatologist accounts. POLL: Do You Post Educational Content for Patients on TikTok?

 Case-Based Roundtable event Lisa Weiss, MMSc, PA-C, offered practical pearls for 3 complex patient cases of atopic dermatitis (AD).

Asa physician associate practicing at the Dermatology & Skin Surgery Center in Stockbridge, Georgia, Weiss led fellow dermatology clinicians in an engaging discussion of real-world examples, best practices for treatment selection, and more.

The cases included a 27-year-old woman with a greater than 20% body surface area (BSA) and severe pruritus, a 36-year-old woman with 8% BSA and AD of worsening severity, and a 7-year-old boy with a 12% BSA and a history of AD since the age of 6 months.

Managing Expectations for Long-Term Therapy

Experts emphasized the importance of managing patient expectations regarding biologics for AD, particularly regarding chronic disease management. They noted that biologics can significantly improve symptoms but are not a cure. The discussion highlighted how treatment often evolves from frequent dosing to longer intervals, aiming for optimal symptom control with minimal medication. Pediatric patients were also discussed, with experts noting that although hope for remission exists, long-term symptom management remains the focus.

Attendees agreed that clinicians should inform patients that although newer treatments may offer significant relief, they may take time to show visible results, typically 4 to 6 weeks. Such discussion is particularly important for those who have experienced previous disappointments with therapies that failed to provide long-term relief.

Attendees highlighted the transition from topical steroids to nonsteroidal treatments such as tapinarof (Vtama; Dermavant Sciences Inc) and roflumilast (Zoryve; Arcutis Biotherapeutics, Inc) as a crucial step in improving patient outcomes, particularly for those with persistent facial erythema. Experts discussed how these treatments offer effective symptom relief while reducing reliance on steroids, providing a safer, more sustainable option for long-term management.

The roundtable also explored strategies for patients who no longerrespond to dupilumab (Dupixent; Sanofi-aventis US LLC and Regeneron Pharmaceuticals, Inc), with experts noting the increasing availability of alternative treatments such as other biologics and Janus kinase (JAK) inhibitors. Although dupilumab remains a key option, clinicians discussed how they would consider switching therapies based on individual patient needs, with a preference for biologics before resorting to JAK inhibitors.

Pearls for Treatment Fatigue and Limited Relief Outcomes

Attendees discussed the importance of considering the individual’s quality of life and treatment goals when managing AD in adults. Clinicians should be mindful of patient concerns, such as treatment fatigue and hesitancy toward long-term steroid use, they noted. Educating patients about nonsteroidal options, such as topical immunomodulators, phosphodiesterase-4 inhibitors, and JAK inhibitors, is crucial for those who experience limited relief from traditional therapies or are concerned about steroid-related adverse effects.

Prioritizing Early, Aggressive Intervention

For clinicians managing moderate to severe AD, early intervention with potent topical treatments can help control symptoms before they become unmanageable. In one patient case, initiating higher-potency steroids early on in the treatment regimen allowed for quicker flare control. However, Weiss agreed it is essential to transition to nonsteroidal therapies as soon as possible to prevent the long-term risks associated with steroid use.

Balancing Outcomes With Impact on Pediatric Growth and Development

Weiss discussed the profound effect that chronic AD can have on a child’s growth and overall development. Persistent sleep disruption due to itch is not just a matter of discomfort; it has been shown to impact a child’s physical growth, she noted. When children, especially those with severe AD, suffer from poor sleep, this can lead to growth restrictions.

Clinicians should recognize the potential link between uncontrolled AD and growth impairment, educating parents that timely treatment of AD can improve both sleep quality and physical development.

Understanding the Emotional and Behavioral Toll on Children

In pediatric AD, clinicians must consider the emotional toll the condition has on both the child and the family, attendees agreed. Children with persistent AD not only deal with physical discomfort but also face emotional challenges, such as frustration, embarrassment, and social withdrawal. These can be exacerbated by sleep deprivation, leading to difficulties in school and social engagement and even causing behavioral problems.

For parents, the emotional burden can be overwhelming, especially when they feel their child is not receiving adequate relief. A key takeaway for clinicians is the need to address the broader emotional and psychological impact of AD on children, discussing how this can affect a child’s ability to focus, learn, and interact with peers. A comprehensive treatment plan that considers physical symptoms and mental well-being is essential.

The roundtable provided valuable insights into the complexities of managing AD, particularly in severe cases across different patient demographics. Key takeaways included the importance of setting realistic expectations for long-term biologic therapy, transitioning to nonsteroidal treatments to improve outcomes, and addressing the psychological toll that AD takes on patients, particularly children.

Beyond Topicals: Surgical Solutions for Vitiligo

While first-line treatments for vitiligo primarily include medical therapies, procedural interventions are emerging as valuable options for patients with recalcitrant or refractory vitiligo. In a discussion at the 

, Nada Elbuluk, MD, MSc, an associate professor of dermatology at the University of Southern California and founding director of their Skin of Color and Pigmentary Disorders Program, provided valuable insights into procedural treatments for vitiligo.

Elbuluk emphasized that "medical treatments for vitiligo are still our first- and second-line therapy," reinforcing the importance of topical immunosuppressive agents, such as corticosteroids, calcineurin inhibitors, and JAK inhibitors, often in combination with phototherapy. These remain the primary treatment strategies for managing active vitiligo. However, when patients exhibit refractory disease—either failing to respond adequately or plateauing in their progress—alternative interventions may be necessary.

For such patients, procedural treatments become a viable option. Elbuluk highlighted the importance of ensuring disease stability before considering surgical interventions, stating, "For these individuals, you also want to make sure they have no new or worsening lesions for at least 12 months." Stability is a crucial criterion, as ongoing disease activity could compromise the success of surgical procedures.

Among the available procedural treatments, melanocyte-keratinocyte transplantation (MKTP) represents the most advanced technique. This autologous procedure involves harvesting melanocytes and keratinocytes from unaffected skin, typically from the lower half of the body, and grafting them onto depigmented areas. "There are new technologies now that allow for this to be done on site, same day, all within a short, confined time period," Elbuluk noted, highlighting the significant progress in making this procedure more accessible and efficient.

As procedural options continue to evolve, Elbuluk expressed optimism regarding broader insurance coverage for these treatments. Expanding accessibility and affordability will be critical in ensuring that patients with stable, treatment-resistant vitiligo have more effective therapeutic options in the future. Updates in Clinical Study of Patients of Color

Mona Shahriari, MD, FAAD, an assistant clinical professor of dermatology at Yale University and associate director of clinical trials at CCD Research, addressed several critical issues in dermatology at the 2025 MOPD conference in Miami Beach. Her insights focused on enhancing patient trust, improving diversity in clinical trials, and addressing disparities in clinical imagery for diverse skin types.

One of the key challenges in dermatologic clinical trials is the lack of trust between clinicians and patients, particularly among individuals from diverse backgrounds. Shahriari emphasized that many patients from these communities have historical reasons for their skepticism, as they were often the subjects of unethical medical practices. Because of this, she advocates for offering clinical trial participation as an option rather than pushing patients into it, allowing trust to build over time.

“A big part of why we have challenges in achieving clinical diversity is that lack of trust between the clinicians as well as the patients,” Shahriari said. “I can’t tell you how many patients come to me because they tell me, 'You look like me, so you must know how to help me.'”

In regard to drug efficacy across different ethnicities, Shahriari noted that there is insufficient data to draw definitive conclusions. She pointed out that many dermatology clinical trials suffer from the underrepresentation of patients from diverse backgrounds, which hampers our understanding of how treatments can perform across ethnicities. She cited a study on secukinumab, which showed lower efficacy in Asian populations and higher adverse events compared to white patients, suggesting a possible difference in drug metabolism.

“That’s why a big push toward having more inclusive clinical trial design… will help us answer those questions,” Shahriari noted.

Shahriari also highlighted efforts to improve clinical imagery for patients with skin of color. During her residency and beyond, she noticed that most textbooks, advertisements, and online dermatology resources featured images of white skin, which posed challenges for diagnosing conditions in diverse populations. She praised initiatives like a trial from Johnson & Johnson, which developed a library of 20,000 images of plaque psoriasis across diverse skin tones, and expressed a desire for similar resources in other dermatologic conditions.

To address the underrepresentation of diverse populations in dermatologic research, Shahriari pointed to ongoing initiatives aimed at making clinical trials more inclusive. She discussed a draft guidance from the FDA aimed at improving eligibility criteria and accommodating patients with transportation or other barriers. However, Shahriari stressed that achieving true diversity in clinical trials is not a simple matter of setting quotas, noting that a comprehensive approach is necessary as the journey toward inclusivity continues.
