Dermatology Times spoke with Paul Yamauchi, MD, PhD, a lead investigator on the trial.

New Research Confirms Effective Therapy for Nail Psoriasis

, Paul Yamauchi, MD, PhD, a board-certified dermatologist at the Dermatology Institute and Skincare Center in Santa Monica, California, spoke about his involvement in a clinical trial evaluating the efficacy of tildrakizumab, a biologic therapy, in treating moderate to severe nail psoriasis. In addition to his private practice, Yamauchi has conducted over 200 clinical trials through the Clinical Science Institute, a research center he founded.

During the interview, Yamauchi emphasized the importance of specific trials for these subgroups of psoriasis patients, stating, "Placebo-controlled trials provide more accurate and reliable data than post hoc analysis."

The study focused on 2 primary endpoints measured at week 28, given that nails grow slowly and require extended observation periods for assessment. The first endpoint was the modified Nail Psoriasis Severity Index (NAPSI) score, which evaluates characteristics such as pitting, nail separation, thickening, and discoloration. According to Yamauchi, a 75% improvement in the modified NAPSI score was observed in 25% of subjects receiving tildrakizumab, compared to only 4.2% in the placebo group The second endpoint was the Visual Index for Severity of Nail Psoriasis (VISNAP), a 5-point scale developed by Sun Pharmaceuticals. Yamauchi explained, "For the VISNAP response at week 28, 29% of subjects attained clear or almost clear nails, compared to a placebo rate of 4.2%." This significant separation between the treatment and placebo groups suggests that tildrakizumab is effective for treating nail psoriasis.

Yamauchi previously investigated tildrakizumab for scalp psoriasis, another high-impact area, which also met its primary endpoints. Yamauchi noted that Sun Pharmaceuticals is planning further trials for genital, hand, and foot psoriasis. He mentioned that topical therapies are typically the first-line treatment for nail and scalp psoriasis. However, if topical treatments fail, systemic options such as pills or injections become necessary. He highlighted tildrakizumab’s advantage, stating, "Since it is administered every3 months, it is very convenient for the patient and a safe product, making it a good choice as a first-line systemic agent."

Safety was another crucial aspect discussed in the trial. Tildrakizumab demonstrated a favorable safety profile, with the most common adverse effects being minor infections. The drug had no significant warnings beyond this, reinforcing its potential as a viable treatment option.

Yamauchi's insights underscore the promising role of tildrakizumab in treating nail psoriasis. The placebo-controlled trial's strong efficacy results, combined with the drug’s safety and convenience, make it a compelling option for patients who do not respond adequately to topical therapies. Additionally, ongoing research may expand its indications to other challenging psoriasis-affected areas, providing further therapeutic options for patients.

Social Media “Hacks”: DIY Sunscreen, Slugging, and More With Chris Tomassian, MD

Chris Tomassian, MD, a board-certified dermatologist and founder of The Dermatology Collective, is passionate about debunking skincare misinformation and educating the public via social media. During an interview with 

, he said one of the most concerning trends he has seen is the rise of DIY sunscreen, where individuals attempt to create their own sun protection at home. He explains that many people mistakenly believe sunscreen is harmful or even cancerous, leading them to seek homemade alternatives. However, as he warns, “People are taking matters into their own hands, and they're like, ‘I could bake bread, so I could bake a sunscreen in my house.’” He emphasizes that commercial sunscreens go through rigorous testing to ensure they effectively block UV rays, whereas homemade versions lack the necessary stabilizers and preservatives, putting users at risk for sunburns and skin cancer.

Tomassian also addresses the broader issue of misinformation in dermatology, particularly on social media platforms like TikTok. He notes that many misleading skin care products gain traction through viral marketing and fake reviews, often promoting unproven claims without scientific backing. This has made it increasingly difficult for patients to differentiate between legitimate, research-backed treatments and trendy but ineffective solutions. “There’s no face behind the product, but it will go viral,” he explains, highlighting how the lack of regulation allows misinformation to spread unchecked.

Another major challenge he faces is the skepticism many patients now have toward established medical advice. While he acknowledges that patient advocacy and research are beneficial, he also finds himself frequently correcting misconceptions, such as the belief that isotretinoin is dangerous despite decades of clinical evidence supporting its effectiveness for severe acne. Additionally, social media has contributed to unrealistic beauty standards through filters and edited images, leading some patients to expect impossible cosmetic results. “They probably had surgery, they had all these things done,” he says, emphasizing that one syringe of filler cannot achieve the dramatic transformations often seen online.

Despite the challenges, Tomassian sees positives in social media, particularly when used to educate and inform. He praises trends like skin minimalism and skin cycling, which encourage a more thoughtful approach to skincare. Looking forward, he plans to expand his practice with a second location in Los Angeles by 2026 and is also exploring new ways to reach audiences. Through these efforts, he continues to push his mission of providing expert dermatological care while combatting misinformation in the ever-evolving world of skincare.

New Biomarker Test Revolutionizes Inflammatory Skin Disease Diagnosis

William Damsky, MD, PhD, an assistant professor of dermatology at Yale School of Medicine, is dedicated to advancing the understanding and treatment of inflammatory skin diseases. As both a practicing dermatologist and a dermatopathologist, Damsky recognizes the significant "practice gap between what's possible in the research laboratory" and what is currently available in clinical settings. This gap, he notes, is due to challenges such as specimen acquisition, processing expertise, turnaround time, and other logistical barriers. His research aims to bridge this gap by developing molecular diagnostics that can seamlessly integrate into dermatological practice.

Atopic dermatitis and psoriasis are 2 of the most common inflammatory skin diseases, yet they present with significant heterogeneity. "One patient with atopic dermatitis may not be exactly clinically or histologically or immunologically identical to the next," Damsky explains. This variability makes treatment decisions complex, as many FDA-approved therapies have distinct mechanisms of action, but there is little guidance on selecting the best medication for individual patients.

To address this, Damsky and his team examined approximately 100 biomarkers, validating 10 before ultimately selecting4 that proved most effective. Their goal was to determine which molecular markers could best distinguish inflammatory skin diseases and predict treatment responses. They explored various laboratory techniques, ultimately settling on in situ hybridization over immunohistochemistry due to its superior ability to detect cytokines, which are critical to disease pathogenesis and targeted by many modern therapies.

A major advantage of Damsky' approach is that it requires no additional effort from clinicians or pathology labs. "The clinician doesn't need to do anything different. They just do a biopsy, as they always would," he explains. The biopsy can be a new or existing sample, allowing retrospective molecular classification of patients. By running the stains overnight using a standardized protocol, results are available the next morning, facilitating rapid and informed treatment decisions.

Damsky envisions a future where molecular diagnostics become routine in dermatology, enabling precise disease classification and more effective use of targeted therapies. This approach could be particularly transformative for rare or severe skin conditions that currently lack FDA-approved treatments. His collaboration with Molecular Instruments has been instrumental in developing RNA probes that preserve tissue morphology and produce clear signals, further enhancing the utility of this technique.

Inflammatory skin disease research is undergoing a "translational revolution," yet clinical practice still lags behind laboratory discoveries. By integrating molecular diagnostics into standard dermatological workflows, Damsky and his team hope to close this gap, ultimately improving patient outcomes and expanding treatment options for a wide range of skin diseases.

spoke with Draelos who shared her excitement for the conference, its sessions, and its speakers. “It's just a great time to make new friends, meet other dermatologists, see where they are in their practice,” she said. “I think going to the AAD, for me, is very invigorating, very rejuvenating, and it’s just fun to see where everybody’s been for the past year.” She shared that this is also a good opportunity to mentor younger clinicians, as the meeting is quite comprehensive.

Draelos was particularly excited about her presentation on nutraceuticals: “I think that’s always a fun session, because we really drill down to, ‘Where’s the science? Where is anti-aging medicine going? Nutritionally, what do you eat to help?’...it’s such a future session, looking at where are we now and what can we do to be sure that we all have quality skin for an entire lifetime.”
