1. What Causes Psoriasis?
Psoriasis is a chronic, immune-mediated disease involving dysregulation of the immune system and persistent inflammation. Rather than normal skin-cell turnover, inflammatory signaling accelerates the growth and accumulation of skin cells, contributing to the plaques and scales characteristic of psoriasis.³
Genetics can contribute to susceptibility, while environmental factors may influence disease onset or flares. Importantly for patient counseling, psoriasis is not contagious and cannot be spread through physical contact.

Patients with psoriasis have increased risks of several associated conditions, including PsA, cardiovascular disease, obesity, diabetes and metabolic disorders, inflammatory bowel disease, and mental health conditions.1,2
This makes the dermatology visit an important opportunity to look beyond visible skin involvement. Clinicians should consider relevant comorbidities and coordinate with primary care, rheumatology, and other specialties when appropriate.
3. When Should Patients with Psoriasis be Screened for Psoriatic Arthritis?
PsA should remain on the radar throughout psoriasis care, not only when a patient spontaneously reports joint pain. The National Psoriasis Foundation (NPF) estimates that approximately 9% to 33% of individuals with psoriasis develop PsA, and some patients may have undiagnosed disease.²
Clinicians can ask about joint pain or swelling, morning stiffness, swollen fingers or toes, heel pain, and changes in mobility or function. Nail disease may also warrant closer attention.
Because untreated PsA can result in irreversible joint damage and disability, identifying concerning symptoms and facilitating rheumatology evaluation can be an important part of dermatologic care.²
4. Does Limited Body Surface Area Always Mean Mild Psoriasis?
Not necessarily.
Body surface area (BSA) is useful for assessing psoriasis, but the percentage of skin involved does not capture every aspect of disease burden. The American Academy of Dermatology (AAD) notes that clinicians can incorporate subjective symptoms and quality-of-life assessments alongside objective severity measures.1
Location matters as well. Psoriasis affecting the scalp, face, genitals, palms and soles, or nails may have an outsized effect on daily functioning, self-esteem, relationships, work, and quality of life even when the total BSA is relatively low.
For NPs and PAs, asking how psoriasis affects the patient's life can provide information that is not apparent from the skin examination alone.
5. When Should Treatment Move Beyond Topical Therapy?
Topical treatments remain important options for many patients with mild to moderate psoriasis, and they may also be used alongside systemic therapies.1 However, treatment decisions should account for more than BSA alone.
Inadequate response to topical therapy, difficult-to-treat or high-impact locations, substantial quality-of-life impairment, extensive disease, or concomitant PsA may warrant consideration of phototherapy or systemic treatment.
Current systemic options include conventional oral agents, targeted oral therapies, and biologics. Treatment selection should be individualized according to disease characteristics, comorbidities, previous treatment, safety considerations, patient preferences, and access.
6. What Should Patients Know About Biologic Therapy?
Conversations about biologics may begin with concerns about injections, adverse effects, immune-system effects, laboratory testing, or the prospect of using long-term therapy.
Clinicians can help patients understand that biologics target specific components of inflammatory pathways involved in psoriatic disease. The AAD-NPF guidelines emphasize patient education regarding treatment risks and benefits and shared decision-making when selecting therapy.1
Rather than simply asking whether a patient is willing to use a biologic, NPs and PAs can explore why a patient may be hesitant. Identifying whether the concern involves injections, safety, cost, monitoring, or another issue can make counseling more productive.
7. Can Patients with Psoriasis Expect Completely Clear Skin?
For some patients, complete or near-complete skin clearance is now an achievable treatment goal.
Clinical trials commonly report Psoriasis Area and Severity Index (PASI) responses, including PASI 75, PASI 90, and PASI 100, representing progressively greater improvement from baseline. PASI 100 indicates complete clearance according to the PASI assessment.
In routine practice, however, the AAD notes that PASI is primarily a clinical trial tool. Measures such as BSA and Physician Global Assessment, along with symptoms and quality of life, can be more practical for evaluating response.1
Setting treatment goals with patients can also help determine whether "success" means complete clearance, relief from itching, improvement in a particularly bothersome area, or simply being able to participate more comfortably in everyday activities.
8. What Can Trigger or Worsen Psoriasis?
Psoriasis triggers vary from patient to patient. Recognized factors that may trigger or exacerbate disease include infections, emotional or physiologic stress, smoking, alcohol consumption, certain medication-related factors, and trauma to the skin.1
The appearance of psoriasis at sites of skin injury is known as the Koebner phenomenon.
Helping patients identify patterns in their own disease may be more useful than providing a universal list of things to avoid. Clinicians should also be cautious not to frame flares as a patient's fault; psoriasis is a chronic immune-mediated disease, and lifestyle modification does not replace appropriate medical therapy.
9. Why Might a Psoriasis Treatment Seem to Stop Working?
A perceived loss of response does not always have a single explanation.
Clinicians can reassess adherence, application technique for topical medications, changes in disease distribution or severity, treatment interruptions, new medications or health conditions, and whether the patient's treatment goals have changed.
For patients receiving systemic treatment, persistent or recurrent disease may warrant reevaluation of the current therapeutic strategy. Shared decision-making can help determine whether optimizing the existing regimen or considering another treatment is appropriate.
10. What Should NPs and PAs Remember at Every Psoriasis Visit?
Look beyond the plaques.
A patient who appears relatively clear may still be experiencing joint symptoms, treatment burden, itching, embarrassment, anxiety about recurrence, or difficulty accessing therapy. Conversely, a patient with visible disease may have priorities that differ from what a clinician assumes based on the examination.
Psoriasis management increasingly involves treating the patient rather than treating a BSA percentage alone. Asking about joints, comorbidities, quality of life, treatment satisfaction, and individual goals can help NPs and PAs identify needs that might otherwise go unrecognized.
During Psoriasis Awareness Month and throughout the year, these conversations provide an opportunity to reinforce a central message of modern psoriasis care: what is visible on the skin may represent only one part of the disease burden.
