The philtrum defines the major architectural interest of the lip and should be in the center of the face and a deep symmetrical “V.” It is important to restore this feature with hyaluronic acid filling. As the lips age, the philtrum becomes flattened and lost giving the female face a more androgenous appearance. Injecting hyaluronic acid with the needle entering at the superior aspect of the philtrum directed inferiorly toward the center of the lip with a nice angle should be the first step in filler lip restoration. This feature can be additionally accentuated by using a lip liner to further define the philtrum. The lip liner will provide definition and prevent the lipstick from bleeding off the vermillion.
