New surgical coding definitions, ambiguous findings can confound

Some of the most common surgical coding errors involve Medicare definition changes, physicians' erroneous assumptions or clinically ambiguous findings, says Howard W. Rogers, M.D., Ph.D., director of Advanced Dermatology, Norwich, Conn.
