Although hair loss is the most visible manifestation of alopecia areata (AA), clinicians increasingly recognize AA as a systemic, immune-mediated condition with consequences that extend far beyond the scalp. Patients often present with fluctuating disease activity, unpredictable progression, and profound psychosocial distress that is not reliably captured by surface-level assessments alone.
At 3 recent Dermatology Times Case-Based Roundtable events, Natasha A. Mesinkovska, MD, PhD; Karan Lal, DO, MS, FAAD; and Arash Mostaghimi, MD, MPH, FAAD, moderated discussions with dermatology clinicians in attendance to review 3 patient cases of AA and Janus kinase (JAK) inhibitor use. Each group discussed how emerging systemic therapies—particularly JAK inhibitors—are reshaping real-world management. Rather than focusing on individual presentations, discussions centered on shared clinical challenges: assessing disease severity, determining when to escalate therapy, integrating clinical trial data into practice, and aligning treatment strategies with patient-defined goals.
Mesinkovska is an associate professor and vice chair for clinical research in the Department of Dermatology at the University of California, Irvine; Lal is a double board-certified pediatric and cosmetic dermatologist at Affiliated Dermatology in Scottsdale, Arizona; and Mostaghimi is an assistant professor of dermatology, medical director of the Dermatology Consult Service, and codirector of the Complex Medical Dermatology Fellowship at Brigham and Women’s Hospital in Boston, Massachusetts.
Across the discussions, moderators agreed that effective AA management requires a shift away from rigid algorithms and toward a more patient-centered framework—one that integrates clinical measures, quality-of-life considerations, and evolving therapeutic data.
Integrating Clinical Trial Data Into Practice
The moderators emphasized the importance of translating trial end points into clinically meaningful conversations. Although Severity of Alopecia Tool (SALT) scores of 20 or less and 10 or less are standard efficacy benchmarks, attendees agreed that patients often define success differently.
“Complete regrowth is ideal, but meaningful regrowth is still success,” one attendee said. “If a patient feels comfortable going out without a hat, that matters.”
Lal and fellow moderators highlighted the need to contextualize trial timelines, reinforcing that regrowth is typically gradual and that early partial responses do not preclude continued improvement.
Attendees also stressed the importance of distinguishing delayed responders from true nonresponders before making treatment changes.
Safety, Monitoring, and Long-Term Management
Safety considerations remain central when initiating systemic therapy for AA. Moderators reviewed baseline laboratory assessments, ongoing monitoring strategies, and counseling approaches related to boxed warnings associated with JAK inhibitors.
Long-term safety data were viewed as increasingly reassuring as real-world experience grows. Adverse events commonly reported in practice, including respiratory infections and laboratory abnormalities, were generally described as manageable with appropriate monitoring.
Patient-Centered Treatment Planning and Therapy Selection
Shared decision-making was repeatedly cited as a cornerstone of successful AA management. Convenience of dosing, comfort with laboratory monitoring, insurance access, and patient expectations were all described as factors shaping therapy selection.
Moderators also discussed scenarios in which switching within the JAK inhibitor class may be appropriate, particularly in cases of inadequate response or tolerability concerns.
Where Deuruxolitinib May Fit
Within the expanding treatment landscape, deuruxolitinib was discussed as an option for patients with severe disease who may benefit from robust and relatively rapid regrowth. Moderators referenced data from the THRIVE-AA trials demonstrating meaningful improvements in SALT scores at 24 weeks.
Importantly, clinicians emphasized that no single therapy is optimal for every patient and that treatment decisions must be individualized.
